Intake Fields (AGCRA cover sheet)
- Date of application
- Student full name, email address, mailing address, and telephone number
- Application year (current scholarship cycle)
- Request type: Initial first year or Renewal for 2nd year
- Applicant category: AGCRA member or Dependent/Grandchild
- AGCRA active membership number
- Current high school/accredited institution details (name, address, telephone)
- Date/projected date of graduation
- Current academic year (Freshman/Sophomore/Junior/Senior or equivalent)
- Payee destination for award disbursement (Registrar/Bursar or established 529 plan)
Required Packet Items
- AGCRA cover sheet
- Membership verification (or VP Membership validation)
- Sponsor signed memo for dependent/grandchild applicants
- Proof of acceptance/admission (or submitted application evidence if pending)
- Most recent transcript
- Volunteering/community service list (dates + descriptions)
- Military affiliation details if applicable (ROTC/prior/current service)
- Leadership endorsements (minimum categories; max 5 letters total)
- Personal narrative (typed, single-spaced, max 2 pages)
- Head-and-shoulders photo with release terms acknowledged
